Ready for Medicare Visit? Here’s How To Prepare

Medicare Wellness Visit is a once-a-year benefit that includes preventative health care. Wellness care is medical attention that aims to reduce the risk of sickness or damage.   • Provide you with a schedule for any additional preventative treatments and screenings that may be needed.
What to Bring to Your Wellness Consultation
The more prepared and structured you are, the more time you and your doctor will have to talk about your health issues. To bring with you to your appointment, it’s a good idea to assemble the following:
  • All medication you take (prescription, non-prescription, vitamins, supplements, and herbal) should be packed in a bag and taken with you.
  • A record of all the healthcare professionals you encounter, complete with names and contact information
  • Your completed health history form and a health risk assessment
